NEED HELP COMPLETING THE FORM?

To fill out the PDFs using Adobe Reader follow these simple steps:

  1. Download Adobe Reader XI (https://get.adobe.com/reader/)
  2. Download both PDFs to your computer
  3. Open Market Application in Adobe Reader XI
  4. Click on Fill & Sign tab in upper right corner
  5. Adding Text: Once you click on Add Text, click the cursor in the section you would like to add information. You will need to click on each line to add information. Your text boxes are moveable if you hover your cursor over the text, see the black arrow, and drag.
  6. Add Checkmark: Once you click on Add Checkmark, you will see a checkmark shadow in place of your cursor, hover the checkmark over the area you would like to mark and left click to place
  7. Once you have filled in all information, be sure to do a Save As of your completed market application. Please save the file as “Your Business Name_MNM Application _2017”
  8. Open Market Rules in Adobe Reader XI
  9. Click on Fill & Sign tab in upper right corner
  10. Place Initials: You will be prompted to set up your initials for immediate and future use. Once you have added your initials you will see an initials shadow in place of your cursor arrow, hover the designated spot and left click
  11. Fill in text boxes using Add Text
  12. Place Signature: Once you have clicked on place signature, you will be prompted to drag a signature box in the spot where your signature will be placed, then prompted to set up a digital signature, select New PKCS#12 digital ID file (you will be able to use for future PDF signatures) fill in all applicable information, click next and assign a password to your signature. To place your signature, you will need to save your Market Rules as “Your Business Name_MNM Rules _2017”
